Community Care  is a service that Veteran Affairs (VA) provides care to Veterans through community providers when the VA cannot provide the care needed. Community care is based on specific eligibility requirements, availability of VA care, and the needs and circumstances of individual Veterans. The VA Community Care Network (CCN) is VA’s direct link with community providers to ensure Veterans receive timely, high-quality care. Telluride Regional Medical Center is a Community Care Network Provider. 

For Primary Care or mental health services, if you are unable to schedule an appointment at a VA health facility within a 30-minute average drive from your home, or if an appointment is unavailable within the next 20 days, you may be eligible to receive primary or mental health care from an in-network community provider. This option ensures you have access to timely, high-quality care when VA facilities are unable to meet your needs promptly. 

Contact TRMC to schedule a visit to apply for VA Community Care to receive authorization for Care at Telluride Medical Center. 

If a Veteran arrives at TRMC for Emergency Care, they will always be provided with care. The VA does need to receive notification of the visit within 72 hours, and the Veteran may contact the VA directly.
