What is Chronic Care Management?

Chronic Care Management helps people better manage their chronic conditions for improved quality of life. Our Care Management Nurses actively work with patients experiencing complex medical conditions, their families and caregivers.

The goal is to teach self-management skills, improve communication between all healthcare providers, and connect people with community resources.

Who is eligible?

Any individual who has Medicare insurance and who has been diagnosed with two or more chronic conditions such as high blood pressure, diabetes, heart disease, high cholesterol, arthritis, emphysema, depression, or cancer is eligible for chronic care management.

Where is Chronic Care Management provided?

Chronic Care Management services include phone calls, portal messages, and office visits.

Your Care Manager works closely with your primary care provider and will help coordinate care with other providers.

The primary aim is to offer support and provide education regarding self-management goals.

How much does it cost?

Depending on your specific plan, you may owe $0, or you may owe a small copay (approximately $2 to $20) each month services are provided.
