 What Should I Know About Well Child Checks?

While it feels like summer has just begun, it's already time to schedule your kiddos' and teens’ annual Well Child Checks before the 2026-27 school year. These visits are an important opportunity to make sure your child is on track with growth and development, immunizations, and healthy habits. They also give families time to discuss any questions or concerns with their child's healthcare provider. 

During a Well Child Check, your provider may: 

  • Track growth and development 
  • Review nutrition, sleep, and physical activity 
  • Discuss school, behavior, and mental health 
  • Perform age-appropriate screenings 
  • Ensure vaccinations are up to date 
  • Answer questions and address any concerns 

Your provider may also request your child to change into a patient exam gown, which provides an opportunity for the provider to perform a more thorough skin exam, check spine alignment, and have a conversation about healthy boundaries. For teens, the provider may also ask the parent/guardian to leave the room to give your child an opportunity to ask questions privately and develop the life skill of being able to communicate with a medical provider.  

To help families prepare for the upcoming school year, Telluride Medical Center has added extra appointment availability in July and August for these important visits. Appointment slots fill quickly, so we encourage families to schedule early. 

Forms: If your child needs a health appraisal form completed for daycare, camp, or another organization, please bring it to the appointment. If your child is planning to participate in activities through the middle or high school, this appointment is the perfect opportunity to complete the Colorado High School Activities Association (CHSAA) pre-participation health evaluation. Per CHSAA, Parents/Guardians are required to fill out the Medical History Form prior to the appointment. (The physical exam and medical eligibility forms will be completed by clinical staff. Only the completed eligibility form should be turned into the school.)

Per CHSAA, the medical evaluation must be performed in person. Health evaluations performed virtually are not acceptable.

Immunizations: If you need your child’s immunization record, you can print it directly from the patient portal (for vaccines administered at TMC), or you can print it out of the Colorado Immunization Information System Public Portal.  

*Insurance typically will only pay for one preventive or wellness check within a 365-day period, so your appointment may need to be scheduled accordingly.
