As part of our partnership with the Colorado Cancer Screening Program, we are working to make sure every patient receives recommended cancer screenings at the right time. 

You may be asked to complete our new Cancer Screening Records Request Form if we cannot find records of a recent breast, cervical, or colorectal cancer screening in your medical chart. The form allows you to tell us where and when you had your screening so we can request records from another provider or facility. 

Keeping your screening records up to date helps your care team know when you are due for future screenings and prevents unnecessary repeat testing. It also helps us provide the best possible care by identifying patients who may need screening referrals.  

Regular cancer screenings can detect cancer early, often before symptoms appear, when treatment is most effective. If you have already completed a mammogram, Pap test, colonoscopy, Cologuard test, or another colorectal screening, please take a few moments to share that information with us. If you have not yet completed the recommended screening, your healthcare team can help you learn about your options.
